SERVICES

 
Our R.I.S.E In-Home Program provides intensive in-home services based on the philosophy that children and adolescents are best cared for within their home and community whenever it is possible. In-Home services are intensive, time limited interventions provided mainly in the residence of the child. Individuals must demonstrate a clinical necessity arising from a severe condition due to mental, behavioral, or emotional illness that results in significant functional impairments in major life activities.

Services are provided in the family home and/or community for children and adolescents up to the age of 21 who are in jeopardy of being hospitalized or referred to an out-of-home placement due to a mental health diagnosis and/or family challenges and has transportation issues. Additionally, children and adolescents who are transitioning home from an out-of-home placement such as foster care, detention, or residential programs may be appropriate for services. Our experienced Community-Based Therapists and Targeted Case Managers help you and your family address behavioral/emotional issues and obtain the right resources.

Holly Hill’s treatment team provides intensive in-home therapy for both the family and the individual utilizing the both individual and family treatment methods. Services are generally time-limited (6 to 9 months), family focused, and delivered in the home. Additional supports include targeted case management services, family support services, and 24 hour a day, 7 days a week availability for crisis stabilization. Services are provided to the identified child as well as other family members in order to enhance skills, ensure continuity of care, and reduce out of home placement.

  • Therapeutic services delivered in the home and community
  • Family and individual interventions
  • Parental coaching and skill development
  • Safety and coping skills development
  • Psychoeducation
  • Case coordination
  • 24 hour crisis support and response
Services begin with a comprehensive diagnostic evaluation completed by a Licensed Behavioral Health Professional. An Individualized Service Plan is developed with the client and family that is used to guide services. The plan is reviewed regularly to evaluate progress and updated as needed. All services are delivered by an independently Licensed Behavioral Health Professional or Behavioral Health Professionals under the supervision of Licensed Behavioral Health Professional.
The purpose of home-based services is to provide comprehensive services to at-risk children, adolescents and families in order to stabilize the family and maintain the home environment, establish and work on treatment goals and facilitate the use of community resources by the family.

We can help families with:
  • Aggressive and/or acting out behaviors
  • Impulse control
  • Anxiety and depression
  • Trauma
  • Family relationships and communication
  • Behavior management
  • Anger management and conflict resolution
  • Life skills (communication, money management, time management, and social skills)
